Register with us as a New Patient

The Practice welcomes new patients and we accept registrations if you live within the practice area. We encourage you to register as soon as you move into the area rather than wait until you are unwell, as records from your previous GP may take some time to be transferred to us.

To register, please come to the surgery where you will be given the relevant forms to complete. If you have a medical card, please bring this with you.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Keeping your records up to date

If you move house it is important that you let us know your new address.

It is also important that we have an up-to-date contact telephone number for you at all times, which we would prefer to be a land-line and not a mobile number.

Patients have a responsibility to keep us informed of their current address and phone number so that we can keep in touch. The best way to do this is to fill in our online Change of a Contact Details Form.

Organ Donation
